EVs are cell membrane-enclosed nanoparticles containing bioactive lipid, protein and nucleic acid molecules that form an intercellular communication network that governs the essential functions of life. These natural biological signals produced by cells directly and indirectly induce specific behavior in other cells. The type and state of the cells that produce EVs influences their biological profile and activity. The lipid, protein and RNA content of circulating EVs reflects the state of the cells that produced them, which provides an excellent opportunity to detect malfunctioning cells in disease and injury. Also, harnessing the innate
bioactivity of some types of EVs such as exosomes enables the development of novel therapeutic agents. As an example, exosomes produced by mesenchymal stem cells (MSCs) have demonstrated the innate ability to suppress inflammation, modulate the immune system, promote cell survival, regenerate diseased or damaged tissues, improve blood supply, reduce scarring and suppress cancer cell proliferation and tumor growth. Bioengineering of MSCs and other progenitor cell lines, as well as the exosomes they generate, further enhances the intrinsic therapeutic value of these unique biopharmaceuticals.

Unique Therapeutic Potential

Exosomes are a subgroup of EVs with a range of sizes between 30 and 150 nm that are thought to be produced in large part by an endosomal pathway. These nanovesicles have demonstrated a significant role in influencing target cell behavior and have innate biological properties that lend themselves to the development of therapeutic agents. Enhancing the inherent immunomodulatory and regenerative capacity of exosomes produced by MSCs and other progenitor cells through bioengineering enables the optimization of this type of EVs for specific clinical applications. Challenges in identification and separation of exosomes in cell

conditioned media and biological fluid samples has led to the development of specialized technology that improves the isolation of exosomes from heterogeneous colloidal suspensions containing EVs and other biomolecules. Cell engineering and EV modification can further facilitate exosome isolation and improve the bioactivity of engineered exosomes, as well as their ability to target specific cells involved in disease and injury. Exosome research with a focus on clinical applications has and will continue to enable the optimization of therapeutic agents for specific clinical indications.

Developing the Potential of EVs

The biological properties of EV will be developed into future diagnostics, more effective therapeutics and innovative regenerative products for improved health and longevity. Examining the biomolecular profile of EVs in biological fluid samples in health and disease using advanced EV analytical techniques and deep learning algorithms will enable earlier and more accurate detection of disease and injury without the morbidity of current diagnostic methods. Bioengineering enhancements to the natural biological activity of EVs will optimize targeted exosome therapeutics for challenging clinical conditions. 

Designing biocompatible carriers and delivery systems will also augment the stability and efficacy of engineered exosomes. Development of simple, safe, effective regenerative products using these combined EV technologies will revolutionize medicine, providing non-invasive solutions to difficult clinical problems. By extension of this technology to disease prevention and general wellness, EV Biologics hopes to change the face of modern diagnostics and therapeutics, as well as general health and wellness.
